Regenemm Healthcare builds AI-assisted clinical documentation tools. We take the safety, fairness, and reliability of our AI systems seriously — and we want to hear from you if something isn't right.
Use this page to report
- a suspected error, harmful output, or unexpected behaviour from a Regenemm AI feature (for example, an inaccurate transcription or generated clinical note);
- a concern about bias, fairness, or inappropriate use of AI in our products;
- a privacy or data-handling concern;
- any other adverse impact you believe our AI systems may have caused to you, your patients, or others.
For suspected security vulnerabilities, please use our responsible disclosure process instead.
Who can report
Anyone — clinicians, patients, carers, partners, team members, or members of the public. You may report anonymously.

Where you can, please include:
- what happened;
- which product or feature was involved (if known);
- when it happened (if known);
- your contact details — optional; leave them out to report anonymously.
Please do not include patient-identifying information in your report. If clinical details are needed to investigate, we will arrange a secure channel with you.
What happens next
Every report is logged, triaged by our Privacy Officer within two business days, and investigated under our AI incident response and nonconformity procedures. If you provide contact details we will acknowledge your report and, where appropriate, tell you the outcome. Reports never result in retaliation.
